"The process of painting a car is highly automated, highly complex and depends on various external variables which are, sometimes, difficult to control. Quality standards regarding paint and finish are extremely high at Audi as these are the most visible features of a car to a customer. Today, it takes years of experience to identify the main drivers of paint failures and keep standards accordingly high. For example, different types of paint require different settings regarding process values and application technique. In order to track the level of quality, every single car is inspected by quality assurance and every failure is documented. For documentation, there are more than 200 predefined types of failures available which are used for standardized documentation. While a car is being painted, data is collected from 2,500 sensors. Those parameters include temperatures, humidity, air flow of application robots, energy consumption, state of filters, etc. All of these variables may influence quality positively or negatively. The challenge of supporting process experts with valuable insights into data is solved by storing sensor data in the data lake and processing the data with Apache Spark and Scala on a HDFS cluster. To identify the most important drivers for paint quality for each failure and each layer of paint, 20 random forest models are being trained daily with MLlib. The results are stored in HDFS and visualized with Tableau. This session will give insights into the challenges of big data at an automotive OEM and how the production of Audi benefits from new big data technologies to make their processes more efficient and raise quality standards even higher. Author: Brendan Gregg Abstract: Traditional systems performance engineering makes do with vendor-supplied metrics, often involving interpretation and inference, and with numerous blind spots. Much in the field of systems performance is still living in the past: documentation, procedures, and analysis GUIs built upon the same old metrics. For modern systems, we can choose the metrics, and can choose ones we need to support new holistic performance analysis methodologies. These methodologies provide faster, more accurate, and more complete analysis, and can provide a starting point for unfamiliar systems. Methodologies are especially helpful for modern applications and their workloads, which can pose extremely complex problems with no obvious starting point. There are also continuous deployment environments such as the Netflix cloud, where these problems must be solved in shorter time frames. Fortunately, with advances in system observability and tracers, we have virtually endless custom metrics to aid performance analysis. The problem becomes which metrics to use, and how to navigate them quickly to locate the root cause of problems. System methodologies provide a starting point for analysis, as well as guidance for quickly moving through the metrics to root cause. They also pose questions that the existing metrics may not yet answer, which may be critical in solving the toughest problems. System methodologies include the USE method, workload characterization, drill-down analysis, off-CPU analysis, and more. This talk will discuss various system performance issues, and the methodologies, tools, and processes used to solve them. The focus is on single systems (any operating system), including single cloud instances, and quickly locating performance issues or exonerating the system. Many methodologies will be discussed, along with recommendations for their implementation, which may be as documented checklists of tools, or custom dashboards of supporting metrics. The recently released Joint Commission Sentinel Event Alert Issue 56 focuses on preventing suicide in health care settings. The new Alert is designed to assist health care providers, including primary, emergency, and behavioral health clinicians, in better identifying and treating individuals with suicide ideation. The Alert also provides screening, risk assessment, safety, treatment, discharge, and follow-up care recommendations for at-risk individuals. The Joint Commission is bringing attention to this issue because its Sentinel Event Database received 1,089 reports of suicides occurring from 2010 to 2014. The most common root causes documented were shortcomings in assessment (most commonly psychiatric assessment). In addition, 21.4% of Joint Commission-accredited behavioral health care organizations and 5.14% of Joint Commission-accredited hospitals, for which a related National Patient Safety Goal was applicable, were noncompliant in 2014 with conducting a risk assessment that identifies specific patient characteristics and environmental features related to suicide risk. Root cause analysis (RCA) is a method of problem solving that tries to identify the root causes of faults or problems. A root cause is a cause that once removed from the problem fault sequence, prevents the final undesirable event from recurring. A causal factor is a factor that affects an event's outcome, but is not a root cause. Though removing a causal factor can benefit an outcome, it does not prevent its recurrence for certain. RCA arose in the 1950s as a formal study following the introduction of Kepner-Tregoe Analysis, which had limitations in the highly complex arena of rocket design development and launch in the United States by the National Aeronautics and Space Administration (NASA). New methods of problem analysis developed by NASA included a high level assessment practice called MORT, and acronym for Management Oversight Risk Tree. MORT differed from RCA by assigning causes to common classes of cause shortcomings, that summarized became a short list. These included work practice, procedures, management, fatigue, time pressure, along with several others. For example, an aircraft accident could occur as a result of weather augmented by pressure to leave on time. Failure to observe weather precautions could indicate a management or training problem, while lack of any weather concern might indict work practices. RCA practice solve problems by attempting to identify and correct the root causes of events, as opposed to simply addressing their symptoms. Focusing correction on root causes has the goal of preventing problem recurrence. RCFA (Root Cause Failure Analysis) recognizes that complete prevention of recurrence by one corrective action is not always possible. This video is targeted to blind users. Tutorial on perf Usage by Hannes Frederic Sowa February 2015 Description from netdev01.org: This tutorial will try to demonstrate the power of performance profiling features within the linux kernel. We will use the networking subsystem as a use case for demonstrating the performance profiling features. The focus will be on the perf framework which has emerged as the de facto standard for performance analysis on Linux. The tutorial will cover well documented as well as hidden gems of the perf infrastructure. We will provide a short overview of perf components and how they work by design. Afterwards the well defined tracepoints in the kernel will be explored, specifically the few available in the networking subsystem. We will proceed to learn how to add custom probes. Often finding a good spot in the kernel to add a probe could be difficult; therefore we will try to demonstrate on how one can try to find a good location for new probes. Modern CPUs are constantly adding more features to their performance monitoring counters, as an example: PMC units were imprecise, so vendors of x86 compatible CPUs improved that situation by adding features to improve precision, called PEBS by Intel and IBS by AMD and what additional data they provide. In the outlook we explore which additional raw counters the CPUs provide and how it is possible to sample events from the memory controller and northbridge and how data is moved around CPU caches (by tracing cache coherency protocols). We will discuss how to write perf scripting to achieve this goal. Participants should be able to understand the PMC documentation provided by the processor vendors and apply them to perf. We will conclude with infrastructure issues around perf, mainly how to deal with debugging information in heterogeneous environments mainly using different and ever changing kernel versions, mismatching debug information and how to deal with that. I hope for insights from the audience because this problem isn't yet solved by us. But the guiding rationale behind this ongoing project has more direct, clinical implications. Addiction and recovery are unfortunately often hamstrung by personal feelings of shame and cultural attitudes of stigma. In this presentation, we intend to explore in-depth the usefulness of applying up-to-date, evidence-based concepts from neuroscience. The aim is to provide clinical tools for reducing the shame and stigma that so often accompany addiction and recovery --- whether for the client in recovery, his/her spouse and family, and even the therapist. Our specific focus throughout this lecture/dialogue is on summarizing and practically applying recent findings from neuroscience about addiction as an empirically documented brain disease. Bio: A highly regarded educator and university administrator, as well as recovery coach, author, and public speaker, Dr. Bob Weathers holds a Ph.D. in clinical psychology, with an M.A. in religious studies. Over the course of his professional career, “Dr. Bob” has provided tens of thousands of hours of therapeutic counseling and recovery coaching to satisfied clients. However, in many cases the causes of such abuses are unknown. Studying packet traces from root servers presents only a part of the picture. We use simulations based on DNS software implementations (BIND8, BIND9, windows*, djpdns) to enhance our understanding of the client-side of DNS transactions. Our lab setup models the typical DNS architecture with root, TLD, SLD, and caching nameservers. We replay a large trace file with different caching software and different network environments. The results advance our understanding of nameserver selection algorithms and the level of DNS traffic injected into the Internet for a given client-side workload. People who perform Random Acts of Kindness generally agree that doing a kind deed for someone else makes them “feel good.” But apart from sheer niceness, why should people be encouraged to commit Acts of Kindness? Are there any other concrete benefits that would motivate more people to become kinder? The answer is an overwhelming “yes!” A number of scientific studies show that Acts of Kindness result in significant health benefits, both physical and mental, for those who perform them. One of the most compelling studies of altruism was performed by Allan Luks and documented in his 1991 book, The Healing Power of Doing Good: The Health and Spiritual Benefits of Helping Others. Luks is the former executive director of the Institute for the Advancement of Health and executive director of Big Brothers/Big Sisters of New York City. Luks’ study involved more than 3,000 volunteers of all ages at more than 20 organizations throughout the country. He sent a 17-question survey to these volunteers, asking them how they felt when they did a kind act. A total of 3,296 surveys were returned to Luks, and after a computerized analysis, he saw a clear cause-and-effect relationship between helping and good health. In a nutshell, Luks’ concluded, “Helping contributes to the maintenance of good health, and it can diminish the effect of diseases and disorders both serious and minor, psychological and physical.” The volunteers in Luks’ study testified to feeling a rush of euphoria, followed by a longer period of calm, after performing a kind act. This feeling, which Luks calls “helper’s high,” involves physical sensations that strongly indicate a sharp reduction in stress and the release of the body’s natural painkillers, the endorphins. This initial rush is then followed by a longer-lasting period of improved emotional well-being. This reduction in stress is vital to the health improvements reported by so many study volunteers. (In fact, more than 90 percent of Luks’ volunteers reported that regular volunteering produced feelings which are a powerful antidote to stress.) Why is stress reduction so important? Because stress can be the root cause of so many maladies, according to Hans Selye, a Hungarian physician who wrote a groundbreaking book called The Stress of Life in 1956. In this book, Selye coined the term “stress,” describing it as a physiological response to external experiences and traumas. Stress causes a racing heart and increased breathing rate, and also sparks the adrenal glands to pump adrenaline into our bloodstream, giving us extra strength. In addition, corticosteroids — which are powerful hormones — and adrenaline work together to release fatty acids into the bloodstream, where they become energy for our muscles. This arousal, if prolonged, shifts from a source of strength and energy into a cause of deterioration. For example, corticosteroids will, over a prolonged period, suppress immune-system functioning. Increased adrenaline and corticosteroids can aggravate diabetes. And as the adrenaline-produced fatty acids needed for energy stay in the blood, the liver converts them into cholesterol, which can lead to arteriosclerosis. A speeded-up heart rate, if prolonged, will increase the risk of high blood pressure, stroke and heart disease. In his book, Selye actually called for a new way of life that would reduce the negative effects of stress — a way of life he dubbed “altruistic egoism.” This way of life would require us to adopt behaviors that involve “the creation of feelings of accomplishment and security [in ourselves] through the inspiration in others of love, good will and gratitude for what we have done or are likely to do in the future.” Selye’s fundamental remedy was to do good for the self by making the effort to do good for others. Selye’s findings on stress are cited by Luks because many of his study volunteers had stress-related health problems that improved after performing kind acts.

PTB Sales' Mechanical Vacuum Pump Refurbishment Facility is located in Los Angeles County, California. It is approximately 17,000 square feet, with 3/4 of this is dedicated by Operations for the remanufacturing of vacuum pumps. In addition the facility stores in excess of 5000 pump cores that can be rebuilt within our standard two week lead time. We also carry a robust selection of equipment that has been rebuilt and is ready to ship, these items can be reviewed anytime from our website. Our Engineering and Technical Staff have over 75 years combined vacuum equipment experience. PTB conducts a proprietary and robust reverse engineering process for supported models. In addition to specific documentation and work instructions, the Bill of Materials replaced during remanufacture is scoped to be on par with the OEM's. Suppliers undergo a rigorous qualification process so that only the highest quality parts are selected for PTB rebuilds. Different than most independent companies, PTB uses sophisticated Production Management software, which creates a Master Production Schedule and the associated Material Requirements Plan. These scheduling tools combined with real-time shop-floor data collection, ensure adherence to scheduled cycle times and quality controls executed on job operations, inventory management, and supply chain management. As each process is completed in the remanufacturing process it is entered into the production management software so that there is up to date traceability for every single order The result is consistent on-time delivery. Remanufacturing process flow: Each pump is accompanied by a traveller to document the steps from Receiving through sub-assembly refurbishment and tests all the way to Final Test and Shipping. These steps are designed to offer our customers flexibility and value while controlling quality. Each pump sent in for repair is verified against the Customer's purchase order for model, serial number, specific configuration and accessories. Pictures are taken and added to the build record. If there are any discrepancies, Customer Service will contact the customer to ensure the order is completely documented as required by the customer. The pumps are then fully disassembled in a ventilated environment. Parts transported to the aqueous parts washer, where they are thoroughly washed, rinsed and high-pressure air-dried. The parts are blasted with Media as specified by the OEM for that Model. After the parts are cleaned, they are inspected for cleanliness, dimensionally verified and examined under 10X magnification for signs of excessive wear or fatigue. Evaluation results are recorded on the shop traveler that accompanies the pump throughout the entire remanufacturing process. Any parts that do not meet PTB's specified quality standards are recorded. We then notify the customer for approval to proceed with the rebuild when additional "hard Parts" are required to be replaced. Motors are tested under required load conditions to ensure that there is the appropriate torque for that specific pump model. In addition, low voltage accessories are tested for functionality and repaired or replaced as required. Computers and smart controllers are electrically and functionally verified, ensuring that commands issued via the controller result in the appropriate responses. Gas management systems are calibrated and verified with their associated electronics. In order to ensure that the cooling system is works at optimum performance, the technician descales the water path, perform a pressure test and a flow test under load conditions. The rebuild kit is pulled from inventory, visually inspected and verified against the work instructions. Any discrepancies are documented, reviewed by an Engineer and corrected. The tech. pulls the assemblies to perform the final assembly. Only after a visual inspection of these to the model's instructions and a review of the build records and QC sign offs does the technician proceed with the rebuild. For required models a helium leak check is performed. All pumps are run through specific series of final performance tests. Upon successful completion, the results are recorded on the Final Inspection and Test Certification sheet that is shipped with the pump. The pump is routed to the paint booth and the to curing, and the appropriate labels are affixed. At Shipping a QC inspection is performed and pictures are taken prior to final packaging. PTB employs a detailed failure analysis process for all pumps returned as warranty claims. Failure Analysis and Corrective Action (FACAR) process: There are built in check points to ensure that the customer reported problem is duplicated, the root cause is identified, the appropriate corrective action has been implemented and the customer is satisfied with the analysis and corrective action.

Demographic changes sweeping the nation and mandates to provide patient centered care present hospitals with new challenges. Substantial patient-provider literature exists documenting patients’ expressed preference/demands for gender, language and race concordance with providers. Research shows increased quality of concordant patient-provider encounters with respect to empathy, decision-making and patient satisfaction. Despite documented advantages, hospitals must carefully balance patient preferences with anti-discrimination laws and hospital diversity/inclusion policies. Making work assignments based on patient preferences for or against certain demographic groups could violate Title VII of the Civil Rights Act. While there are certain Bona Fide Occupational Qualifications (BFOQ) exceptions to anti-discrimination laws, such as religious stipulations related to modesty, no clear guidelines exist on how staff should respond to concordance requests when patient accommodations qualify as a discriminatory practice. Providing concordance accommodations for any characteristics, including gender, race and ethnicity, and language, while seemingly innocent and in accordance with the standard of patient centered care, can cause significant problems for hospitals, clinics and individual providers. First, keeping track of patient requests and attempting to modify schedules and work assignments based on those requests substantially complicates patient care for healthcare administrators. In some cases, patient health could be compromised due to care delays caused by waiting for a provider with the “correct” demographic characteristics to be available. A larger problem may be the effect on hospital employees themselves, in relaying the message that they are not adequate in certain settings. This presentation lays out a clear set of guidelines for how hospitals should handle all patient concordance requests. • All patient concordance requests need to be addressed in a respectful but firm way, clearly explaining which requests will and will not be accommodated • Reasonable accommodations can only be made if they include BFOQ exceptions and can be supported by indisputable clinical, spiritual and emotional benefits. Accommodation requests based on personal preferences which could be influenced by explicit or implicit bias are not appropriate • If accommodations are to be made, they should be done in a way that minimizes interference with typical hospital protocol • Providers should consult with bioethics and legal experts when handling concordance requests. They should also make every effort to understand the patient/family’s rationale for making such a request. By following the guidelines laid out in this presentation, healthcare organizations can successfully navigate patient preferences while complying with laws and policies designed to foster diversity & inclusion for all.

This is our second opportunity to produce and document a video during a “TalkBack” after a screening of the award winning film, ”Indian Point”, directed by Ivy Meeropol. This particular screening was a special showing organized as a benefit for Hudson River Sloop Clearwater by Manna Jo Greene, the Environmental Director of the organization, who moderated and participated in the “TalkBack”. Ivy Meeropol, the director of the film led the discussion in which Roger Witherspoon, an environmental journalist, and Marilyn Elie, a local activist with the Indian Point Safe Energy Coalition (IPSEC), also participated. Both Roger and Marilyn appeared in the film. This Q&A session is jam packed with information about: the current status of Indian Point, how much electricity Indian Point contributes to our grid and much more. The myth that Indian Point, or it’s owner, Entergy, actually produces 25% of NYC’s energy needs is debunked by Roger Witherspoon, as he walks the audience through how much electricity the plant produces and how much the New York City/ Westchester grid uses. This is something the film does not go into. Other points covered include: Indian Point is operating without a license and was not required to do a root cause analysis when 22% of the baffle bolts performing a key function were found to be degraded. This is the largest percentage of degraded bolts ever found in a reactor anywhere. And this in spite of Indian Point’s potential to affect the 20 million people who live and work within the 50 mile danger zone with Indian Point as its center. Ivy Meeropol answers questions about her choices in directing the film. She makes clear that the film does not take sides. This applies to the music she selected and had written for the film and how it is used in the film. She lays out the issue through the eyes of the people she follows and let’s viewers come to their own conclusions. In response to an audience member who wanted more timeline graphics and more detailed information about plant closures, Meeropol restates her commitment to the intelligence of her audience and the power of story. There are plenty of advocacy films on nuclear power. This documentary is unique because it is not one of them. It stands in a class by itself and the talk after the screening details just how. Indian Point, the documentary, is a character driven story of nuclear power – both for those who favor it and those who want to close the reactors at Indian Point. The story is told through the eyes of Brian Vangar, Control Room Supervisor, Gregory Jasckzo, former head of the Nuclear Regulatory Commission, Roger Witherspoon, environmental journalist, and Marilyn Elie, advocate for closing the plant. Can’t talk about root of ethics without talking about the root of Metaphysics as they are simply the two sides of the same coin. The Adepts/ Mahatmas are keepers of the Divine Wisdom on which the root if ethics and metaphysics is based. Problem is the illusion we work under which is that we are all separate. Acting from a basis of separateness generates Karma and isolation. Science and religion try to go back to the cause but they tend to stop at the level of Metaphysics and claim that no more exists or perhaps call the rest God. Concretized version of the material God idea adopted from the Jewish Religion. The kernel of Theosophy in the realm of Metaphysics was explained and are documented in HPB’s Psychic and Noetic Actions. Discussion of our will and how we interpret that condition based on our senses, a basis that we must get beyond. Man is a microcosm of the macrocosm. Theosophy per the original Secret Doctrine is a perfect system for understand analysis and correspondences between life in the cosmos and life at the human, animal, plant, and mineral levels. We are influenced by the various grades of material in the Astral and the material world that mask the real. We can overcome this mask via analogy and correspondence.. The highest point of visible light is Akâsa. Matter and substance and how they are to be viewed are discussed. As long as we don’t go beyond our 5 senses, we will not perceive and develop our higher two senses. All effects on every plane are the results of causes set in motion. We are influenced by the Astral Light. Law governing elementals and the possibility that understanding them opens up channels toward truth. Evolution toward elevation of our thoughts, deeds and actions. We must reach toward 1. securing the highest of man’s Supreme nature, 2. excavation of selfishness, ,cultivating the inner via Mediation, control of appetites, performance of duty, 3. training (spiritual, intellectual, physical by which higher faculties are aroused and developed and 4. Adeptship and beyond So, we must take our own evolution in hand and then we get helped.

We have document control issues. I've done training, we have procedures, we've had countless NCR's, I've literally audited people's computers, and I don't know how to handle this anymore. It is quite honestly, one of the most frustrating things since it seems so damn simple. During my first production internal audit of the year I found that when the designers send down a new drawing (model) to our programmers, they didn't get rid of the older version of the model. This is standard procedure, if you check other job files, it is not this way, they are organized properly and consistently, but alas the one I was auditing, this was what I found. What is insanely frustrating is that everyone should know what document control is. The designers especially should know what document control is. We had one of our NCR's from our recent transition audit for document control. I just don't know what the next step is. How do I do an NCR and close it properly when the root cause literally seems like "this person is lazy". - I am working with a company to assist them in transitioning from the old standard to the new one. I have identified the risks that and will address them through the use of an FMEA analysis. Likewise, I have also identified opportunities through the use of SWOT, but how would I go about helping the organization figure out which opportunities it should address? Company B is taking over Company A on Apr 2017 and business year end June 2017. 1) How to create an integration MR into Company B. Company A last MR is on Sept 2016. 2) Is it necessary to create two management review? Appreciate your advise. Thank you. - Considering that the current standard doesnt require too much documented records. how do you manage the next issue? in a procedure its is mentioned a document which is an evidence that one product is available in the storage. But this document is not included in the QMS, (not in the list of documents, nor list of records) is somewhat informal, but anyway gives you traceability of such product. Do all the documents mentioned into the procedures should be registered into the QMS?
